Ethics approval and consent to participate
: Multi-site ethics approval was obtained prior to commencement of the trial and for subsequent protocol amendments via the North West Lancaster NHS Research Ethics Committee (reference 14/NW/1113) who are independent of all the institutions involved and had no role in the design, conduct or reporting of the research. Local NHS research governance approvals were also obtained at the two sites (UK Clinical Research Network (UKCRN) portfolio ID: 17309). All participants provided written informed consent to participate.
