: This study was approved by the University of Wollongong Human Research Ethics Committee (HE13/277). Children who were older than 16 years of age provided their own consent to participate in the study, children under 16 years of age acquired parental consent and gave assent prior participating in the focus groups. Verbal consent was obtained from staff who chose to be involved in the focus groups.
