: The establishment of the BTB sample collection, the study database and the patient information and consent forms were approved by The Swedish Ethical Review Authority (Dnr 2021-04058, 2015/1092-31/4, 2014/75-32, 2011/1932-32, 2011/315-32, 2010/1182-31/2).
