: This study was approved by the Pathfinder internal review committee and by the ethical committee at the National Institute of Health of Mozambique in 2015, protocol ID 01/CNBS/2015. The authors of this study obtained the formal permission from all participants and the voluntary character of this study was highlighted. They were informed that they could refuse to answer specific questions and or refuse all questions at any point of the study. Confidentiality was guaranteed related to identity and personal information. The authors have used the terms “Upper Province” and “Lower Province” to ensure protection of the identities of stakeholders within the two study sites.
