: All participants provided voluntary, written informed consent, and where participants were under 18 years of age, parental consent and individual assent were obtained. Ethical approval was obtained from the Kenya Medical Research Institute (KEMRI) in Kenya (KEMRI/SERU/CCR/0028/3240), and the Human Research Ethics Committee (HREC) of the University of Witwatersrand in South Africa (M160131). In Kenya, additional support and approval was obtained from the Kiambu County Health Research and Development Unit and participating healthcare facilities.
