: The study protocol was approved by the Makerere University School of Medicine Research and Ethics Committee (Ref# 2014-052) and the University of California, San Francisco Human Research Protection Program, Committee on Human Research (IRB# 12-09573 and IRB# 15-17467) and the Uganda National Council for Science and Technology (REF#:1541212101). All study participants underwent an informed consent process.
