The Institute of Public Health, Obafemi Awolowo University Health Research Ethics Committee gave ethical approval for the study (IPH/OAU/12/1604). The parents of participants aged 11–17 years gave written informed consent for the participation of their wards in the study. Respondents aged 18–23 years old gave independent written informed consent for study participation. Study participants were informed about the purpose of the study, the freedom to withdraw at any time, and were assured of confidentiality. There was no reimbursement for study participation.
