: The anonymous Survey of Perception data collection was considered public health surveillance rather than research by the CDC, waiving the need for formal informed consent, as determined by the CDC contract and agreed upon by the University of Florida Institutional Review Board. (Note: Only the collection of Survey of Perception data was waived from consent, other data collection such as our Health Needs Assessment required IRB approval; those data are not included in this manuscript). All sites asked participants before beginning if they wanted to participate and because of the anonymous non-research-based protocol, signatures were not allowable. Only persons who verbally agreed to participate were surveyed. All aspects of this work adhered to the Helsinki Declaration; everything that we do at the University of Florida or any university partner site complies with the declaration.
