: The study was approved by the University of Toronto Health Sciences Research Ethics Board (protocol #41749). The University of Toronto (U of T) was the primary site of the study. Participants were recruited by community collaborators who were based within community-based organizations in Ireland, UK and US and Canada, who did not have access to an institutional Research Ethics Board. The protocol, including our international community engagement and recruitment strategy, was reviewed in the context of the University of Toronto Health Sciences Research Ethics Board. All interview data were collected and stored at U of T. During the consent process, potential participants were informed that U of T was the primary site of the study and the location where data would be collected and stored, and that U of T’s Health Sciences REB had approved the study protocol. The study was performed in adherence to the Declaration of Helsinki. All participants provided informed consent (either written or verbal).
