: The study received ethical approval from the University of Bristol’s Faculty of Health Sciences research ethics committee (ref. 10517). This study was performed in line with the principles of the Declaration of Helsinki. Informed consent was obtained from all individual participants included in the study. Participants were provided with a participant information sheet about the study which detailed what we will do with the data and explained their right to withdraw from the study, and gave written consent before taking part. For the observations, we provided those attending with written information in advance and the facilitator informed people again on the day that the researcher was present, but that they did not have to talk to them unless they wanted to.
