: This study was approved by the Institutional Ethics Committee of the George Institute for Global Health (Project Number 05/2019) which adhers to the Declaration of Helsinki. Before initiating the project, due permissions were obtained from the Department of Health and Family Welfare (DHFW) Kerala.
