: This study was approved by the University of Central Florida Institutional Review Board (#00005558), and the study protocol was performed following the relevant guidelines and regulations. All procedures performed in this study involving human participants were in accordance with the ethical standards of the institutional research board and with the 1964 Helsinki Declaration and its later amendments or comparable ethical standards. An end user agreement was obtained from the Protocol for Responding to and Assessing Patient Assets, Risks, and Experiences (PRAPARE, www.nachc.org/prapare) to use their survey in this study. Prior to survey completion, patients were required to complete an informed consent document indicating the purpose of the study and their consent to participate. Informed consent for patients younger than 18 years of age was obtained from their guardian on their behalf.
