: This study adhered to the national guidelines for conducting health research in Nigeria, and the research protocol was implemented in accordance with all relevant guidelines, and regulations. Ethical approval for the study was obtained from the Health Research Ethics Committee of the University of Nigeria Teaching Hospital Ituku-Ozalla, Enugu, Nigeria. The ethical approval number is NHREC/05/01/2008B-FWA00002458-IRB00002323. Also, permission to conduct the study was obtained from the Enugu State Ministry of Health. Written informed consent was obtained from all individual participants included in the study.
