: This study was a secondary analysis of NDHS data, used with permission from the Demographic and Health Programme. The protocol was approved by the Ethics Committee of the ICF Macro at Calverton in the USA in conjunction with the National Ethics Committee of the Federal Ministry of Health in Nigeria (ethics approval number: NHREC/01/01/2007), in accordance with the Nuremberg code and the Declaration of Helsinki.
