: The Ethical Committee of the Helsinki and Uusimaa hospital approved the Finnish Public Sector study (HUS/1210/2016) in accordance with the Declaration of Helsinki. In the Finnish Public Sector study informed consent is received from the participant by informing them in the cover letter of the questionnaire that the responses would be used in scientific research and answering the questionnaire is voluntary and if one does not wish to participate in the study, to ignore the questionnaire or notify the researchers of refusal.
