Ethical approval was granted by the Health Research Ethics Committee of the University of Nigeria Teaching Hospital, Ituku-Ozalla, Enugu (Approval No.: NHREC/05/01/2008B-FWA00002458-IRB00002323). Additional ethical clearance was also obtained from the Ebonyi State Health Research Ethics Committee.
