Ethics approval and consent to participate
: The data collection was approved by the Danish Data Protection Agency as part of University of Southern Denmark’s joint registration (Journal no. 2015–57-0008). Patients were informed about the project and their rights and gave consent that data are used for research purposes. According to Danish law the study did not require ethical approval (Research Ethical Committee for Region of Southern Denmark journal no. S-20162000-109).
