: This study was conducted to the guidelines of the Declaration of Helsinki of 1975, as revised in 1983 and approved by the local ethical committee (The Norwegian Regional Committee for Medical and Health Research Ethics (2015/89)). Informed consent was obtained prior to study participation from all participants included in the study authorizing the use of demographic and clinical data collected as part of this study. Patients and clinicians did not receive any compensation for participating, which aligns with the standard practice in Norway.
