: Since this was not a planned study but instead a post-hoc decision to collate standard records maintained by a home for orphaned and abandoned infants, it was not deemed necessary to apply to an official research ethics review however a letter of ethics waiver was provided by the Biomedical Ethics Research Committee at the University of KwaZulu Natal on 26 July 2018.
: Permission for the information to be used for scientific purposes was provided by the legal guardians at the home (ITL ref. 001/18) and all data has been anonymised.
