Ethics approval and consent to participate

: Ethics approval was obtained from the University of North Carolina institutional review board (13–1005) and the Human Research Ethics Committee at the University of Witwatersrand (130123). Consent to use programmatic data was systematically obtained for all clinic clients and documented in patient files. Women declining use of routine clinic data for research purposes were not included in this analysis. Written informed consent was obtained for all IDI participants, and all interviews were conducted in private spaces to ensure confidentiality.
