Ethics approval and consent to participate
: Study protocols were approved by the ethical review committee of The University of Sargodha, Pakistan [UOS/17/Acad/399]. We obtained official permission to conduct interviews from the District Health Authorities in each district. All participants received a structured letter outlining study aim, procedure, the right to withdraw, and a promise of anonymity. Study objectives were thoroughly discussed with each participant, and written informed consent was obtained before starting interviews.
