: Ethical clearance for the study was obtained from the ethics review committee, Faculty of Medicine and Allied Sciences, Rajarata University of Sri Lanka (ERC/2017/28). Participants were provided with a full explanation about the purpose, general content and the objectives of the study and informed written consent was obtained from mothers and verbal consent was taken from PHMs. The administrative clearance to conduct the study was obtained from the Regional Director of Health Services, Anuradhapura.
