Ethics approval and consent to participate
: This study obtained ethical approval from the Higher Degrees Research and Ethics Committee of Makerere University School of public Health (protocol 624) and was registered by the Uganda National Council for Science and Technology (HS 2477). Permission to undertake the study was also obtained from the district authorities. Study participants provided written informed consent and their privacy and confidentiality was ensured.
