Ethics approval and consent to participate: This study was approved by the University of Michigan Institutional Review Board (HUM00288513). Hospitals will designate, in writing, their decision to join the IGNITE-IOL trial. An oral consent script will be reviewed with healthcare workers at the start of LEAD and TEAM. Patient consent was waived because the study involves only secondary analysis of patient medical data.
