: This study was based on a database analysis over a 5-year period (2007–2013). While obtaining patient-level consent was not possible in this study, an application was made and approved in accordance with the Queensland Laws, Public Health Act 2005, Sect. 284 (PHA approval no. RD005208 obtained on 11 August 2014). Additionally, data was de-identified and coded (to remove any potentially identifiable confidential information) prior to any statistical analysis.
