: This retrospective study was accepted by the administration of Kuopio University Hospital and Wellbeing Services County of North Savo (IRB approval number 200/2017). The study followed the Declaration of Helsinki and was performed according to Good Clinical Practice (GCP) guidelines. Patients enrolled in the prospective clinical trial provided their written informed consent prior to participation. Ethical committee approval was not required for the retrospective registry component of the study.
