: All patients (or their legal representatives) provided written informed consent before enrolling in the TOSCA disease registry. The study was designed, implemented, and reported in accordance with the principles of Good Clinical Practice, Good Pharmacoepidemiology Practices of the International Society for Pharmacoepidemiology (ISPE 2008), the STROBE (Strengthening and Reporting of Observational Studies in Epidemiology) guidelines, and the ethical principles laid down in the Declaration of Helsinki, and all local regulations. : All patients were informed that the reason for enrolling in the study was to collect natural history data for scientific analysis and publication (an obligation mandated by the Good Clinical Practice guidelines) and they signed consent forms with that understanding.
