: The study design and related documents were approved by an independent review board in July 2016 and February 2017. Individuals provided verbal informed consent prior to participation in the interview and audio-recording; participants invited others (e.g., family members) to participate in their interviews if they chose. Participants in the CDIs were offered compensation of US$150.00 for their time and an additional US$50.00 for travel expenses as needed for in-person interviews.
