: Approval for the conduct of this study was received from the Lebanese University (date: 6th of February 2017) and Médecins sans Frontières (number 1616) Ethical Review Boards.
: Informed consent was sought from each participant of the cross-sectional survey. Specific consent to publish was not sought from the additional patients who were included in the retrospective analysis of routine clinical data.
