Ethical approval and consent to participate
: The study was approved by the ethics committee of the National Public Health Faculty from the University of Antioquia, the Harvard T. H. Chan School of Public Health IRB and The Hospital for Sick Children (SickKids). Participants completed and signed an informed consent and their confidentiality was respected by using anonymous codes that made personal identification impossible. At the request of the ethics committee, no mention shall be made of the names of the two municipalities in the case study, in order to protect the identity of the community and the participants.
