: The study was conducted in accordance with the principles of the Declaration of Helsinki and Brazilian ethical guidelines for research of the Conselho Nacional de Saúde (Brazilian National Health Council). Approval for the data used in this study was acquired from the Comissão Nacional de Ética em Pesquisa (Brazilian National Commission for Ethics in Research; CONEP) (protocol numbers 2.689.528 and 6.468.159), and the Comitês de Ética em Pesquisa (Municipal Research Ethics Committee; CEP) (protocol number 6.210.448). As the study utilised fully anonymised data obtained through secure linkage, the requirement for informed consent was waived by the approving ethics committees (CONEP & CEP). The authors had full access to all anonymised databases employed in this analysis.
