Ethics approval and consent to participate
: All procedures in this study were carried out in accordance with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. The proposal of the FU-study was submitted to the institutional review board (IRB) of GGzE, the Institute of Mental Health Care. On 15 January 2013, the IRB concluded that this study was in accordance with the prevailing medical ethics in the Netherlands. In addition, they declared that the study did not fit the conditions of the Medical Research Involving Human Subjects Act and, therefore, that no additional examination by a medical ethical committee was required for this study. Written informed consent was obtained from each participant.
