: The study was granted exemption from requiring ethics approval by the Medical Ethics Committee of the Leiden University Medical Center. The Committee declared that ethical approval was not required as the study intended to improve care as usual. The protocols are in accordance with the ethical standards of our institution and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. Verbal or written informed consent was obtained from all individual participants included in the study. For participants under the age of 16, legal guardians also provided (verbal) informed consent.
