Ethics approval and consent to participate
: The study was approved by the Regional Committee for Ethics at Karolinska Institutet – Dnr: 2014/1067–31/3.All patients were verbally informed of the design and nature of the study and that their identities would be protected. They gave permission to record the interview, and later, the audio recordings were locked in a safe. The patients received written information in which it was made clear that their participation was voluntary and that they could withdraw from the study without any repercussions to their clinical follow-ups. The patients were asked to sign a written consent form.
