: The study was approved by the Scientific and Ethics Review Unit of Kenya Medical Research Institute. Permission to conduct the study was granted by the Ministry of Health via the county health offices and the hospital review boards before the study commenced. To protect the confidentiality of the patients and the integrity of the medical records as per the clinical research code, data were coded using unique identifiers, and access thereof was limited to the study team. The codes were linked to medical records and the link was maintained through a paper log kept at the hospitals.
