: Plasma samples were collected from high-risk heavy-smoker volunteers (age 50–75 years), including current or former smokers with a minimum pack/year index of 30 enrolled in a LDCT screening trial performed at our institution (BioMild Trial, ClinicalTrials.gov: NCT02247453)[]. EVs were also isolated from the plasma of lung cancer patients affered to Thoracic Unit of our Institute. Plasma collection was approved by the Internal Review and the Ethics Boards of the Istituto Nazionale Tumori (INT 11–21) of Milan. All patients provided informed consent.
