: Ethical approval was not required. The NZ Health Information Privacy Code states that identifiable health information can be disclosed to a researcher under certain conditions; the disclosure is for a directly related purpose (i.e. audit for quality of service). In the case of the data provided for analysis, all identifiers were removed and could not be re-linked to the individual practitioner.
