The consent to participate in the study was requested from parents and subsequently kept and made available for any needs. Furthermore all methods were carried out in accordance with the ethical standards as laid down in the 1964 Declaration of Helsinki and its later amendments or comparable ethical standards.
: The questionnaire that was used indicated that completion of the survey would represent the participants’ consent for their replies to be used for publication of study results. No additional consent procedure was required.
