: The BOX trial was approved by the regional ethics committee of the Capital Region of Denmark prior to initiation (Approval no. H-16033436). Inclusion of incapacitated patients was performed after consent from a trial guardian in the acute setting in accordance with Danish legislation. Relatives were asked for consent as soon as possible, and patients who regained consciousness were also asked for consent.
