Ethics approval and consent to participate: The study was designed and conducted following the declaration of Helsinki guidelines and received ethics approval from the Royal Brisbane and Women’s Hospital Human Research Ethics Committee (HREC/13/QRBW/1) as the lead site. All other participating sites also obtained individual HREC approval for the SMARRT study. All non-SMARRT-funded studies that contributed data also obtained individual HREC approval at each participating site. All patients or their legal representatives provided written informed consent before enrolment.
