: The Ethics Committee at the University of British Columbia approved all the experiments using human tissue resources. Patients in Vancouver, British Columbia, were recruited and samples were collected under the tumor tissue repository (TTR-H06-00289) as described [], except Pt6 (SA1035), which was collected under REB-approved protocols at the McGill University Health Centre and Jewish General Hospital as described [].
