Ethics approval and consent to participate
: The study has been reviewed by North Wales Research Ethics Committee (reference 14/WA/1211). Practices managers are asked to sign a contract with UEA on behalf of all practice staff. Patients will not be required to provide consent to use their data as their extracted data are anonymised. The trial is registered on the ISRCTN registry: reference number ISRCTN95472706.
