: Ethical approval was obtained from the South Central Oxford C REC (17/SC/0034) and the CAG (17/CAG/0015). Study participants are healthcare organisations who provided informed consent to participate through signing a contract. Use of routine electronic healthcare data from patients admitted to these organisations was approved by REC without individual consent from these admitted patients.
