Ethics approval and consent to participate
: The study was reviewed and approved by London Queens Square Ethics Committee (ref. 17/LO/0934) in September 2017. All subsequent substantial amendments to the protocol have been reviewed and approved by the ethics committee and Health Research Authority (HRA) and all non-substantial amendments were notified to the HRA. The trial has been registered in the ISRCTN Registry (trial ID: ISRCTN44340629) and is conducted in compliance with Good Clinical Practice standards and principles of the Declaration of Helsinki (2013).Written prospective or retrospective informed consent will be obtained from all participants.
