Ethics approval and consent to participate
: Ethical approvals for this research have been secured from the National Center for Disease Control and Public Health of Georgia (IRB # 2018–01) and Queen Margaret University, Edinburgh, UK (IRB # REP0172). All managers of health care facility networks or managers of stand-alone facilities involved in the trial will sign a consent form. Informed consent will be obtained from patients as they enrol into the study (see Additional file).
