Ethics approval and consent to participate
: Central ethical approval for all study activities has been confirmed from the Institutional Review Board (IRB) at Center on Addiction (Approval #240), and we will not begin recruiting at other research sites in the trial until local ethical approval has been obtained. All study activities will be subject to monitoring by the Data Safety and Monitoring Board (DSMB) of the same institution (see Additional file ). Any modifications to the protocol that may impact the conduct of the study or its specified objectives and procedures will require a formal amendment to the protocol and approval by the IRB and DSMB prior to implementation. Consent from participating therapists will be obtained in confidentiality by research staff (see Additional file ).
