Ethics approval and consent to participate

: The protocol was approved by the University of Auckland Human Participants Ethics Committee in accordance with the Declaration of Helsinki (021888) dated 20 September 2018. Approval is granted until 20 September 2021. Recruiting will not begin at other centers in the trial until approval from those sites has been obtained. Informed consent will be obtained from all study participants. Prior to registering for the study, participants are asked to acknowledge that they have read and understood the Participant Information sheet and Consent Form (see Additional file InternalRef removed) before being able to proceed with the online screening forms. Participants, trainers and key stakeholders will receive separate Participant Information sheets and Consent Forms for the interviews and are asked to acknowledge either online or in writing that they have read and understood these forms prior to the interview. Any study protocol modifications to the study objectives, methodology, or the participant process will result in a formal protocol amendment sent to the University Ethics Committee for approval.
