Ethics approval and consent to participate
: This study was reviewed and approved by the Regional Committee for Medical and Health Research Ethics (REC) in Mid-Norway (2015/902). The study will be conducted according to the Declaration of Helsinki and ICH-Good Clinical Practice guidelines and follow the CONSORT rules [ , ]. Protocol modifications will be reviewed by REC and Clinical Trials. Parents must give written informed consent on behalf of themselves and their children in order to participate. In addition, children 8 years and older will be informed of the study.
