Ethics approval and consent to participate
: Ethics approval was obtained from the School of Medicine institutional review board at Makerere University College of Health Sciences and the Uganda National Council for Science and Technology at the beginning of the study and renewal of approval was sought for the follow-up study. Participants consented for both the initial assessment and the 1-year follow-up at the beginning of the study. Only consenting participants were included. During the follow-up study participants were included only if they were willing to continue. Participants could not be identified by the information they provided.
