Ethics approval and consent to participate
: This study will be conducted according to the principles of the Declaration of Helsinki (version 7, October 2013) and the Guidelines for Good Clinical Practice and in accordance with the WMO and the Dutch law in general. Ethical approval has been obtained from the accredited MREC of The Netherlands Cancer Institute – Antoni van Leeuwenhoek (MREC AVL, reference number M17GSC / NL62356.031.17) for the original protocol (December 14, 2017) and all amendments. In all participating sites additional local approval was obtained. Written informed consent will be obtained from every participant before inclusion in the trial. Patients will be informed about and subsequently included in the trial by the treating physician or nurse. Patients are asked for permission to be approached for participation in follow-up studies on the informed consent form.
